Adding a form to group forums...need help!

How would you add forms to forums within groups? would i use php? or is there a simpler way?

Hi Mike

My friend has a site that I work on and she wants to add a form to her group that is on the site. The form would allow users to fill it out and submit the it to her. At the moment, she has a forum post that asks users to send certain information to her via a message. I thought it would be easier to just include a form within the group's forum that could be submited via a message but all they would have to do is fill in the text boxes and click submit.
